Required Documents Prior to Death
In order to create your end-of-life wishes, you will need to fill out and have notarized
the'Five wishes' pamphlet or the Colorado form 20 or a Will stating end-of-life
wishes. You will also need to write down the following:
Vital Statistics:
1) Full legal name
2) Legal address, City, State and Zip Code and Mailing address if different
3) Sex, Race and Country of Citizenship
4) Birth date and Birthplace
5) Marital status ... name of Spouse or Maiden name. Date and Place of Marriage.
If spouse is deceased, date and place of death.
6) Education 0-12 years and College, also number of years
7) Social Security Number
8) Occupation; Type of business or industry
9) Father's name; Mother's maiden name
10) Veteran? If so give dates of service
11) Name of designee or executor
Additional documents may include:
* Living Will
* Durable Power of Attorney
* Advanced Health Directives
PLEASE inform family memberes, a close friend, physician or business associate
about the location of your documents.
If you wish to be cremated by the Crestone End of Life Project, present a Statement
verifying that you have discussed your wishes with next of kin and that they support
your decisions.
Required Documents Following Death
* Death Certificate
* Authority for Final Disposition Certificate
Traditionally, mortuaries handle such documents. Those families choosing to act as
funeral directors of the deceased will need to obtain and fill out these documents.
